In order to be prepared for your arrival on campus, we ask that you complete all forms pertaining to your year of athletic eligibility (Incoming Freshmen/Transfer or Returning Athlete). Please return those forms in an addressed stamped envelope to the following address:

These forms will provide us with the pertinent information related to your medical history, which will allow for a more thorough and efficient physical examination. Each of the following forms and documents must be completed and returned to the Northeastern University Sports Medicine Department by all student-athletes. If you have sustained an injury prior to arriving at Northeastern University, you may need additional diagnostics tests before allowed to participate in a sport. The athlete and/or parent are financially responsible for any diagnostic testing.

Also, be advised that a physical and proper immunization are required for all incoming freshmen and transfer students by the University Health and Counseling Center and is to be returned to the University Health and Counseling Center – not the Sports Medicine Department. The University Health and Counseling Center physical needs to be completed whether or not you participate in a sport, however, the physical is not to be considered a substitute for any other physicals which may be required to participate in a sport.
